Certificate of Origin Policy
1. Certificate of Origin service will be performed free of charge for members in good standing up to three times per calendar year. For additional requests, a fee of $20.00 per certificate will be charged. The fee for non-members is $50 per request.
2. Certificates of Origin must be filled out completely and notarized before being presented to the Chamber.
3. Any supporting documentation (i.e. manufacturer's invoices) must be submitted for inspection along with the Certificate of Origin. A copy of the Certificate of Origin and supporting documentation is to be provided for the Chamber's files.
4. While the Chamber will attempt to perform Certificate of Origin services in a timely manner, same day service cannot be guaranteed.
